ABSTRACT

AIM: To systematically evaluate the efficacy of intense pulsed light(IPL)combined with meibomian gland expression(MGX)in the treatment of meibomian gland dysfunction(MGD)-related dry eye disease(DED).METHODS: Chinese and English databases such as Chinese national knowledge infrastructure(CNKI), Wanfang, VIP, CBM, ClinicalTrials, PubMed, Embase and Web of Science were searched, and clinical randomized controlled trials(RCTs)using IPL combined with MGX in the experimental group and MGX alone in the control group from January 2017 to September 2022 were included. Six outcome indicators including clinical efficacy, ocular surface disease index(OSDI)score, break-up time(BUT), corneal fluorescein staining(CFS)score, tear meniscus height(TMH)and meibomian gland yielding secretion score(MGYSS)were Meta analyzed by Review Manager 5.3 and Stata 14 software.RESULTS: A total of 15 RCTs were included, with 1 345 patients with MGD-related dry eye. Meta-analysis results showed that the treatment of MGD-related dry eye in the experimental group improved better clinical efficacy(OR=4.95, 95%CI: 2.76~8.90, Z=5.35, P<0.00001), BUT(SMD=1.26, 95%CI: 0.84~1.69, Z=5.78, P<0.00001), TMH(SMD=0.37, 95%CI: 0.15~0.59, Z=3.33, P=0.0009), and reduced OSDI scores(SMD=-0.86, 95%CI: -1.44~-0.27, Z=2.85, P=0.004)as well as MGYSS(SMD=-2.43, 95%CI: -4.31~-0.54, Z=2.52, P=0.01)than the control group. However, there was no statistically significant difference in CFS scores(SMD=-0.19, 95%CI: -0.46~0.07, Z=1.43, P=0.15).CONCLUSION: IPL combined with MGX in the treatment of MGD related dry eye can increase the overall effective rate and improve the symptoms and signs of patients with MGD related dry eye better than MGX alone.

ABSTRACT

Cardiovascular diseases(CVD) with high morbidity and mortality pose severe threats to human life. Allicin, a main active ingredient of garlic, possesses multiple pharmaceutical activities. It not only exerts cardioprotective effects but also prevents the risk factors for CVD. Allicin exerts cardioprotective effects via a variety of mechanisms, including inhibiting oxidative stress, apoptosis, autophagy, and inflammatory responses, regulating lipid metabolism and gut microbiota, inducing hydrogen sulfide production, and dilating vessels. Despite the valuable cardioprotective effects, the instability of allicin has hindered the basic research and clinical application. This paper reviews the progress in the cardioprotective effects and mechanisms of allicin in the last decade and summarizes the methods to improve the stability of allicin. In addition, this review provides a reference for further research and development of allicin in cardiovascular protection.

ABSTRACT

Glaucomatous optic neuropathy(GON)is the difficulty of glaucoma treatment. In recent years, a variety of theories have been put forward about the pathogenesis of GON, but none of them can explain the principle of optic neuropathy caused by all types of glaucoma, which makes the disease difficult to treat in clinical treatment and is not conducive to early intervention. The latest research found that the transient receptor potential channel vanillic acid subfamily 4(TRPV4)in the retina plays an important role in various pathogenesis of GON. This article will review TRPV4 and its role in the occurrence and development of GON in order to find a common “connection point” for the multiple mechanism theories of GON, which will contribute to further understanding and clinical treatment of the disease.

ABSTRACT

Objective:To explore the effects of long non-coding RNA (lncRNA) FAM224A on the proliferation and migration of ovarian cancer cells by regulating the expression of miRNA-590-3p (miR-590-3p).Methods:Human ovarian cancer cell lines OC3, SKOV-3, HO-8910, A2780 and human normal ovarian epithelial cell line IOSE80 were selected, and the relative expression of FAM224A in each cell line was detected by real-time quantitative polymerase chain reaction (qRT-PCR). The cell line with the lowest relative expression level of FAM224A was screened for follow-up experiment. The cells were divided into FAM224A group (transfected with FAM224A mimic gene) and control group (transfected with control mimic gene). CCK-8 method and cell scratch test were used to detect the cell proliferation and migration ability of the two groups. The bioinformatics website LncBase v.2 predicted that the target gene that FAM224A might complementarily bind to was miR-590-3p. qRT-PCR was used to detect the relative expression levels of miR-590-3p and forkhead box protein A2 (FOXA2) mRNA, and the expressions of related proteins were detected by Western blot.Results:The relative expression levels of FAM224A in ovarian cancer cell lines OC3, SKOV-3, HO-8910, A2780 and normal ovarian epithelial cell line IOSE80 were 0.23±0.04, 0.65±0.05, 0.45±0.03, 0.63±0.08 and 1.02±0.11, respectively, and the difference was statistically significant ( F = 14.78, P < 0.01), and the cell line with the lowest relative expression level of FAM224A was OC3. The results of CCK-8 method showed that the proliferation ability of OC3 cells in the FAM224A group was lower than that in the control group on the 2nd, 3rd, 4th and 5th day of culture (all P < 0.05). The scratch healing rates of OC3 cells in the FAM224A group and the control group were (18.6±2.3)% and (71.7±7.2)%, respectively, and the difference was statistically significant ( t = 6.99, P < 0.01). The relative expression levels of FAM224A in OC3 cells in the FAM224A group and the control group was 12.36±1.45 and 1.14±0.24, respectively ( t = 13.08, P < 0.01); the relative expression levels of miR-590-3p were 0.19±0.06 and 1.04±0.20, respectively ( t = 4.01, P < 0.01); the relative expression levels of FOXA2 mRNA were 6.37±1.37 and 1.05±0.08, respectively ( t = 3.86, P < 0.01). Compared with the control group, the expression of FOXA2 protein in OC3 cells in the FAM224A group was increased, the expressions of cell proliferation protein cyclin-dependent kinase 2 (CDK2) and cyclin D3 were decreased, and the expression of cell migration protein Snail was decreased. Conclusions:FAM224A is low expressed in ovarian cancer cell lines. FAM224A reduces the proliferation and migration ability of ovarian cancer OC3 cells by inhibiting the expression of miR-590-3p.

ABSTRACT

AIM:To investigate the correlation between severity of diabetic retinopathy(DR)and corneal sub-basal nerve plexus(SNP)changes. METHODS: There were 132 patients with 132 eyes in type 2 diabetes(T2DM)and 80 patients with 80 eyes in age-related cataract selected in our hospital from January 2018 to May 2021, among them, there were 52 non-diabetic retinopathy(NDR)patients with 52 eyes, 40 non proliferative diabetic retinopathy(NPDR)patients with 40 eyes and 40 proliferative diabetic retinopathy(PDR)patients with 40 eyes in T2DM. The general data and corneal laser scanning confocal microscopy were analyzed. Spearman rank correlation analysis was used to evaluate the correlation between DR clinical stage and nerve fiber length. RESULTS: There were no differences in gender and age comparisons among 4 groups(P&#x003E;0.05). The diabetes duration in PDR group was significantly longer than NPDR group and NDR group(P&#x003C;0.05). The diabetes duration in NPDR group was significantly longer than NDR group(P&#x003C;0.05). The levels of fasting blood glucose and glycosylated hemoglobin in age-related cataract group were significantly lower than other three groups(P&#x003C;0.05). The best corrected visual acuity in PDR group was significantly lower than NPDR group and NDR group(P&#x003C;0.05). The best corrected visual acuity in NPDR group was significantly lower than NDR group(P&#x003C;0.05). The length of nerve fibers in age-related cataract group was significantly higher than NDR group, NPDR group and PDR group(P&#x003C;0.05). The length of nerve fibers in PDR group was significantly shorter than NPDR group(P&#x003C;0.05). Spearman rank correlation analysis showed that there was a negative correlation between DR stage and nerve fiber length(rs=-0.347, P&#x003C;0.001).CONCLUSION: There was a correlation between the severity of DR and the changes of corneal SNP. The length of nerve fibers in patients with PDR was significantly shorter than patients with NPDR; Both PDR and NPDR have a loss of neural structure and should pay attention to the evaluation and treatment of ocular surface lesions in the treatment of T2DM fundus lesions.

ABSTRACT

Based on theory of "paired relationship between the heart and the small intestine" in traditional Chinese medicine(TCM), heart disease can affect the small intestine, and intestinal diseases can also affect the heart. The heart controls mental activities, so if the secretions function of small intestine is impaired, it will hinder the mind of the mind. Modern medicine has found that intestinal flora is closely related to cognitive function, which is consistent with the "paired relationship between heart and small intestine" in TCM. Diabetes cognitive impairment (DCI) is a category of "diabetes dementia" in TCM. According to TCM, the imbalance of spleen and stomach is the starting factor of diabetes dementia, and the phlegm turbidity of spleen deficiency is the pathological product of diabetes dementia. Previous projects have proved that intestinal flora disturbance is the modern pathological basis of spleen and stomach imbalance in TCM, suggesting that restoring spleen and stomach function to adjust intestinal microbial homeostasis is helpful for the prevention and treatment of diabetes dementia. At present, many achievements have been made in the study of the mutual causality between diabetes and intestinal flora disorder. In recent years, the incidence of DCI has increased rapidly, and the pathogenesis is complex and unclear, which increases the difficulty of treatment. Therefore, finding new targets should not be ignored. Given the complex connections among intestinal flora, diabetes and cognitive impairment, it is particularly important to tease out the relationship among the three. Based on previous scientific research, the author analyzed layer by layer and found that intestinal flora can not only affect the synthesis of neurotransmitters in the brain, but also participate in the common pathogenesis of diabetes and cognitive impairment such as chronic inflammation, insulin resistance and blood glucose rise, with a typical target effect. The pathogenic mechanism of intestinal flora in DCI was summarized in order to establish a new target and direction for the treatment, and to explore the scientific connotation of TCM in regulating spleen and stomach for treatment of diabetes dementia.

OBJECTIVES@#To study the effect of glucose metabolism disorders on the short-term prognosis in neonates with asphyxia.@*METHODS@#A retrospective analysis was performed on the medical data of the neonates with asphyxia who were admitted to 52 hospitals in Hubei Province of China from January to December, 2018 and had blood glucose data within 12 hours after birth. Their blood glucose data at 1, 2, 6, and 12 hours after birth (with an allowable time error of 0.5 hour) were recorded. According to the presence or absence of brain injury and/or death during hospitalization, the neonates were divided into a poor prognosis group with 693 neonates and a good prognosis group with 779 neonates. The two groups were compared in the incidence of glucose metabolism disorders within 12 hours after birth and short-term prognosis.@*RESULTS@#Compared with the good prognosis group, the poor prognosis group had a significantly higher proportion of neonates from secondary hospitals (48.5% vs 42.6%, @*CONCLUSIONS@#Recurrent hyperglycemia in neonates with asphyxia may suggest poor short-term prognosis, and it is necessary to strengthen the early monitoring and management of the nervous system in such neonates.

ABSTRACT

Objective: To analyze and predict the Q-marker of Cinnamomi Ramulus in Danggui Sini Decoction based on fingerprint and network pharmacology. Methods: The fingerprints of Cinnamomi Ramulus Decoction and Danggui Sini Decoction were established, and analyzed by using the similarity evaluation system software for chromatographic fingerprint of traditional Chinese medicine (2012 edition); The network pharmacology was used to screen and analyze the function target and pathway of related components of Cinnamomi Ramulus, and the "component-target-pathway" network was constructed to predict the potential Q-marker of Cinnamomi Ramulus in Danggui Sini Decoction. Results: The fingerprints of 15 batches of Cinnamomi Ramulus Decotion and 15 batches of Danggui Sini Decoction were established. The similarity of fingerprints was more than 0.96, and seven common components were identified, including protocatechuic acid, coumarin, cinnamic acid, cinnamaldehyde, cinnamyl alcohol, 2-methoxy cinnamic acid, and 2-methoxy cinnamaldehyde. A total of five active components, seven core target sites and 15 key pathways of Cinnamomi Ramulus were screened out through network pharmacology system, and based on the "Five Principles" of quality markers, 2-methoxy cinnamaldehyde, cinnamaldehyde, and cinnamic acid were predicted as potential quality markers. Conclusion: In this study, the quality markers of Cinnamomi Ramulus in Danggui Sini Decoction are analyzed by fingerprint and network pharmacology, which provides a basis for comprehensive control of the quality of Danggui Sini Decoction, reference for further study on the mechanism of Danggui Sini Decoction, and demonstration for the correlation study of quality markers of compound and single medicine in the classic prescription.

ABSTRACT

SUMMARY OBJECTIVE: To investigate clinical curative effects of lumbar oblique-pulling manipulation in combination with sling-exercise-therapy training on chronic nonspecific lower back pain. METHODS: A total of 60 patients with chronic nonspecific lower back pain in the Outpatient Department were included in this study. These patients were randomly divided into two groups: the observation group and the control group. The control group adopted a single sling-exercise-therapy training three times a week, while the observation group adopted lumbar oblique-pulling manipulation in combination with manipulation treatment once a week. The course of treatment lasted for four weeks. RESULTS: (1) Before and after treatment, the ODI score was compared within the group. A remarkable statistical significance was observed from the third day (P<0.05). At the third month of follow-up, the difference in ODI scores between these two groups was statistically significant (P<0.05). (2) Before and after treatment, it was observed that differences in VAS scores from the third day were statistically significant (P<0.05). (3) The difference in muscle strength between these two groups had remarkable statistical significance in the third month of follow-up (P<0.05). CONCLUSION: The effective rehabilitation function of lumbar oblique-pulling manipulation in combination with sling-exercise-therapy training in patients with CNLBP is superior to that of sling-exercise-therapy training alone.


RESUMO OBJETIVO: Investigar os efeitos curativos da manipulação lombar com o movimento de puxar oblíquo combinado a terapia por exercícios de sling-training para dor lombar baixa crônica não específica. METODOLOGIA: Um total de 60 pacientes com dor lombar baixa crônica não específica no ambulatório foram incluídos neste estudo. Esses pacientes foram divididos aleatoriamente em dois grupos: o grupo de observação e o grupo de controle. O grupo de controle aderiu apenas à terapia por exercícios de sling-training três vezes por semana, enquanto o grupo de observação aderiu à manipulação lombar com o movimento de puxar oblíquo combinado à terapia por exercícios de sling-training uma vez por semana. O tratamento durou quatro semanas. RESULTADOS: (1) Antes e após o tratamento, o escore de ODI foi comparado no grupo. Uma significância estatística notável foi observada a partir do terceiro dia (P<0,05). No terceiro mês de acompanhamento, a diferença nos escores de ODI entre os dois grupos foi estatisticamente significante (P<0,05). (2) Antes e após o tratamento, observou-se que diferenças nos escores de VAS a partir do terceiro dia foram estatisticamente significantes (P< 0,05). (3) A diferença de força muscular entre os dois grupos apresentou significância estatística notável no terceiro mês de acompanhamento (p<0,05). CONCLUSÃO: A função de reabilitação efetiva da manipulação lombar com o movimento de puxar oblíquo combinada à terapia por exercícios de sling-training em pacientes com dor lombar baixa crônica não específica é superior à da terapia por exercícios de sling-training sozinha.

ABSTRACT

Objective: To observe the effect of Shenqi compound recipe on glucose and lipid metabolism in patients with Qi and Yin deficiency and blood stasis syndrome newly diagnosed type 2 diabetes mellitus (T2DM), and its intervention effect on intestinal microecology and serum proinflammatory factors. Method: The 106 eligible patients were divided into the observation group (54 cases) and the control group (52 cases) by random number table method. Another 40 healthy volunteers in physical examination center of the hospital during the same period were enrolled as health control group. On the basis of Guidelines for the Prevention and Treatment of Type 2 Diabetes in China(2013 edition), control group was provided lifestyle interventions, such as reasonable diet, weight control, moderate exercise, salt restriction, tobacco control, alcohol restriction and psychological balance. In addition to the therapy of the control group, the observation group was given Shengi compound for oral administration, 2 times/days. Both groups were treated for 8 weeks. The fasting blood glucose (FBG), postprandial 2 h blood glucose (PBG), glycosylated hemoglobin (HbA1c), homeostasis model assessment-insulin resistance index (HOMA-IR), total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) before and after treatment were evaluated. The structure and quantity of intestinal flora before and after treatment were detected. The traditional Chinese medicine(TCM)symptom was scored. The levels of interleukin-1β (IL-1β), interleukin-6 (IL-6), IL-8, and tumor necrosis factor-α (TNF-α) were measured before and after treatment. Result: FBG, PBG, HbA1c and HOMA-IR levels in observation group were lower than those in control group (PPPPβ, IL-6, IL-8 and TNF-α levels in observation group were lower than those in control group (PZ=2.134, PConclusion: Shenqi compound can regulate blood glucose and blood lipid in patients with newly diagnosed T2DM (Qi and Yin deficiency and blood stasis syndrome), improve IR, intestinal microecology imbalance, and reduce non-specific inflammatory response, with a good clinical efficacy on intestinal microecology of patients with Qi and Yin deficiency and blood stasis syndrome newly diagnosed type 2 diabetes mellitus.

ABSTRACT

Aim To observe the protective effects of cordycepin ( Cor) on dopaminergic neurons in 1-meth-yl-4-phenyl-1 , 2 , 3 , 6-tetrahydropyridine ( MPTP )-in-duced mouse model of Parkinson's disease ( PD) and to explore its mechanism. Methods C57BL/6 mice were administered with MPTP to establish the PD mod-el. Mice in Cor groups were pretreated with Cor (2.5,5.0 and 10.0 mg·kg-1 ) by intragastric administra-tion, respectively. The motor functions of the mice were observed in the open-field test, rotarod test and pole test. The content of DA, the numbers of TH-im-munoreactive cells and apoptotic cells were measured respectively by HPLC-ECD, immunohistochemistry staining and TUNEL staining. The expression of apop-tosis related proteins and MAPK signaling pathway-re-lated proteins ( p38 , p-p38 , ERK1/2 , p-ERK1/2 JNK1/2 and p-JNK1/2 ) were determined by Western blot. Results Cor could significantly improve the mo-tor dysfunction in PD mice. The contents of DA, DOPAC and HVA in the striatum remarkably increased after administration of Cor in MPTP-induced mice. Mo-reover, Cor could obviously reduce both the loss of TH-immunoreactive neurons and the numbers of TUNEL-positive cells in substantia nigra pars compacta ( SNpc) of PD mice. The protein expressions of Bax and cleaved caspase-3 were markedly down-regulated,whereas those of Bcl-2 and the ration of Bcl-2/Bax were significantly up-regulated by Cor pre-treatment followed by MPTP treatment. Furthermore, the protein expressions of p-p38 , p-ERK1/2 and p-JNK1/2 signif-icantly decreased in substantia nigra in Cor groups. Conclusions The results suggest that Cor can protect DA neurons against MPTP-induced injury by inhibiting apoptosis, which may be closely relevant to the inhibi-tion of MAPK signaling pathways.

ABSTRACT

Obstructive sleep apnea-hypopnea syndrome (OSAHS) is a common clinical disease that seriously threatens human health and life. Accurate location of the upper airway obstruction is the key to the diagnosis and treatment of OSAHS. Acoustic pharyngometry uses sound reflection to quickly assess the cross-sectional area and volume of the upper airway. Acoustic pharyngometry represents a simple, quick, non-invasive method for measuring upper airway dimensions which could predict sleep apnea risk. In this article we sought to introduce the application of acoustic pharyngometry in the diagnosis and treatment of OSAHS.

ABSTRACT

Cordycepin(3′-deoxyadenosine),a derivative of the nucleoside adenosine,is a major functional component of Cordyceps militaris. It has been found that cordycepin possesses a variety of pharmacological activities,such as antibacterial,antivial, antiinflammatory,antitumor,immunoregulation,hypolipidemic,and hypoglycemic activities. In recent years,the effects of cordyce-pin on the central nervous system(CNS)have attracted great attention,and it has been found that cordycepin could not only affect the function of the CNS but also protect nerves from injuries. This paper reviews the effects of cordycepin on sedation and hypnosis ,the im-provement of learning and memory,and the protection of nerve injuries caused by cerebral ischemia/hemorrhage ,nerve toxin(gluta-mate,β-amyloid,rotenone and 6-hydroxydopamine,etc.),lipopolysaccharide and trauma,along with the in vitro toxicity as well as acute and subacute in vivo toxicity,so as to offer valuable references for future study and application of cordycepin.

ABSTRACT

Objection To investigate the main influencing factors for extracting total polysaccharides and two active adjuvant components from Poria cocos. Methods Extracting temperature was designed between 20℃-100℃,ratio of material/water was 1/5-1/20 (W/V),extracting time was 1-6 h,extracting frequency was 1-3 times,material size was crude slices~50 mesh,and extracting mode was standing or stirring. The sugar content was determined by phenol-sulfuric acid method and the reletive molecular mass of polysac-charides was determined with gel filtration chromatography. Monosaccharide compositions were analyzed with PMP-derivation and cap-illary electrophoresis. Results ①The extracting temperature should be below 70℃;②The ratio of material/water was suitable be-tween 1/5-1/20;③The extracting time might be within 2 h;④The extracting frequency should be no more than two times;⑤The raw material should be crushed before extracting;⑥The stirring was favorable for extracting polysaccharides. Conclusion The re-sults indicate that the extracting temperature,extracting frequency and stirring mode are important for extracting polysaccharides,and the cushing of the raw materials is helpful to extract the active adjuvant component of polysaccharides from P. cocos in higher yield.

Objective To explore the effects of chitosan flocculation clarification process and alcohol precipitation process on the principal chemical constituents of water extract of Codonopsis Radix. Methods The influence of two purification processes on water extract of Codonopsis Radix was investigated through lobetyolin contents, extract yield, and relative apparent content of each component in HPLC fingerprint as evaluation indexes. Results Chitosan flocculation clarification process showed a significantly higher extract yield of water extract compared with alcohol precipitation process, and it has a markedly better retention effect for strong polarity constituents; alcohol precipitation process exhibit a little better retention effect for lobetyolin and a better retention effect for weak polarity constituents. Conclusion The above two processes have some regularity in the influence on the main chemical constituents in the water extract of Codonopsis Radix, which can provide some guidance for the reasonable choice of the purification process for water extracts of Codonopsis Radix, and other TCM water extracts.

ABSTRACT

Artemisia rupestris is a traditional medicine in Uygur and Kazak in Xinjiang Province, mainly distributed in the territory of Xinjiang Altai area, Tianshan mountains and the Kunlun mountains, growing at an altitude of 1 500 to 4 000 meters of grassland and forest areas. As the broad research on chemical constituents, pharmacological activity, the effective components of A. rupestris have attracted the interest to make up new drugs. Based on the latest research from A. rupestris, identification and geographic distribution, chemical constituents, pharmacological effects, clinical applications were summarized in this article, in the view of Medicinal Ethnobotany. At the same time, some suggestions were proposed for future research.

ABSTRACT

Objective To study the effect of gradient pressure socks combined with intermittent pneumatic pressure pump on lower extremity deep venous thrombosis after gynecological laparoscopy.Methods Toally 45 patients to underwent gynecological laparoscopy during April 2013 to February 2015 were assigned as observation group and from April 2012 to March 2013 another 45 ones to undergo the same therapy were as control group.The latter was treated with routine nursing and the former with gradient pressure socks combined with intermittent pneumatic pressure pump apart from the routine nursing.The effective rate was conpared between two groups.Result The observation group was significantly more effective in the prevention of lower extremity deep venous thrombosis than that of the control group (P<0.05).Conclusion The gradient pressure socks combined with intermittent pneumatic pressure pump can be effective in the prevention of lower extremity deep venous thrombosis after gynecological laparoscopy and thus deserves clinical popularization.

ABSTRACT

Objective: To discuss the postoperative analgesia efficacy of multimodal analgesia of ropivacaine combined with dezocine, and to illuminate the feasibility of multimodal analgesia in the children undergoing cheiloplasty.Methods:In the randomized, controlled and double blind study, sixty children scheduled for cheiloplasty were randomly divided into ropivacaine group,dezocine group and multimodal analgesia group (n=20). The children in ropivacaine group and multimodal analgesia group were treated with infraobital nerve blockade (1.5 mL 0.25% ropivacaine)before skin incision.The children in dezocine group received the same volume of normal saline. The patients in dezocine group and the multimodal analgesia group received dezocine (0.15 mg·kg-1 )20 min before the end of operation, and the children in ropivacaine group received the same volume of normal saline.The children’s ages and weights,duration of anesthesia and operation, reviving and extubation time,agitation score and incidence,laryngospasm or bronchospasm,CRIES scores at 2,4,6,8,12, and 24 h after operation and adverse reactions were all recorded.Results:There were no significant differences in the age,weight,the duration of anesthesia and operation of the children between three groups (P >0.05).Compared with ropivacaine group,the reviving and extubation time of the children in dezocine group and multimodal analgesia group were increased (P 0.05).There were no laryngospasm or bronchospasm occured in all groups.The CRIES score at 2 h after operation of the children in multimodal analgesia group was the lowest and there were significant differences compared with other two groups (P 0.05).There were no significant differences in the CRIES scores at 8,12,and 24 h after operation between three groups (P > 0.05).Compared with other two groups,the incidence of tachycardia and the cases using analgesic in multimodal analgesia group were the lowest,and there were significant differences compared with other two groups (P < 0.05 ).There was no respiratory inhibition in all groups.Conclusion:The multimodal analgesia of ropivacaine combined with dezocine can effectively prolong the postoperative analgesia duration and reduce adverse reactions, and it can be safely used in the postoperative analgesia in the children undergoing cheiloplasty.

ABSTRACT

Objective To analyze the clinical characteristics, predisposing factors and prognosis of tuberculosis (TB) patients after allogeneic hematopoietic stem cell transplantation (allo-HSCT), and to summarize the clinical treatment experience. Methods The information of 7 confirmed TB patients after allo-HSCT, including the onset time, symptoms, laboratory findings, imaging findings, treatment and prognosis, was collected, and their clinical characteristics and regulations were summarized. Results Seven of the 481 patients who received allo-HSCT were found to have TB, with an post-transplantation TB incidence rate of 1. 46%. The seven patients included 2 females and 5 males, ranging 18-46 years and averaging (31.3 ± 9. 6) years old. Five patients had pulmonary tuberculosis (2 with hematogenous disseminated pulmonary tuberculosis), 1 had tuberculous pleurisy, 2 had tuberculous meningitis, and 1 had bladder tuberculosis. The clinical manifestations included fever, cough, urinary tract symptom, and headache. Three cases had history of chronic graft versus host disease(GVHD) while one of them experienced acute GVHD. PPD test and tuberculosis antibody test were negative in all the 7 patients: peripheral blood T-SPOT test was positive in 3 cases, with one having positive bronchoalveolar lavage fluid smear. Biopsy results showed granulomatous inflammation in 2 cases, with 1 accompanied by coagulative necrosis. Chest CT showed patched shadow in 3 cases, miliary nodule shadows in 2 and pleural effussion in one. Six patients were clinically cured after anti-tuberculosis treatment and one died, with an effective rate of 85.7%. Conclusion TB infection should be considered once the patients receiving allo-HSCT have infectious symptoms, and early screening should be done to avoid misdiagnosis and missed diagnosis. Early diagnosis and routine anti-TB treatment can yield good outcome.

OBJECTIVE: To examine the hearing status of pre-job workers exposed to noise in metal manufacturing industries,and to analyze the related influencing factors. METHODS: A judgement sampling method was adopted to choose1 597 pre-job workers exposed to noise in metal manufacturing industries as the study subjects. The data of workers' occupational health examination was collected,and the influencing factors of hearing loss were analyzed. RESULTS: Among the 1 597 individuals,631( 39. 5%) cases reported occupational noise exposure history( noise exposure group) with the length of service of 0. 20( 0. 10-0. 30) years; 966( 60. 5%) cases denied occupational noise exposure history( no-noise exposure group). The hearing loss detection rate was 46. 1%( 737 /1 597); among them,the occupational contraindication detection rate was 13. 9%( 222 /1 597),and the detection rate of other kinds of hearing loss was 32. 2%( 515 /1 597).The detection rates in workers with occupational contraindication and the other kinds of hearing loss in noise exposure group were higher than those in the non-noise exposure group( 16. 5% vs 12. 2%,50. 6% vs 43. 3%,P < 0. 05). The male workers had higher incidence than female workers [the odds ratio( OR) were 1. 958 and 2. 331,respectively,P < 0. 01);the workers with age > 25 years had higher incidence than those with age ≤ 25 years( OR were 2. 390 and 2. 245,respectively,P < 0. 01); the noise exposure workers had higher incidence than non-noise exposure ones( OR were 1. 536 and 1. 287,respectively,P < 0. 05). CONCLUSION: Pre-job occupational health examination is helpful for early detection of occupational contraindication in noise exposure workers. Attention should be paid to the male workers,the older workers,and those with occupational noise exposure history when conducting occupational health examination.
